We ask that if there is any reason that you cannot keep a scheduled appointment, please give us at least 48 hours notice by phone. In the event that you do not appear for your scheduled appointment, we will unfortunately be forced to charge you a fee for which you will be responsible.

If you have dental insurance, we will be happy to submit your claim. We will provide all the information your insurance company needs to get you the maximum allowable benefits available. Please remember that we have no contract or relationship with your insurance company and that payment for treatment is the responsibility of the patient, regardless of insurance limitations.
